Same here but it was Fairbanks with a carmi IN Calling me and toll free to call back. Same number that another user posted! Computer generated voice. Blocked the 317-343-9312 number!

Received a voicemail from Samantha with the Indiana Department of Health wanting to conduct an important survey on the health of US residents. Stayed that she will will call again in the next few days to conduct the interview. Called from an Indianapolis area code, but left the 855 number to call if I had any questions.

This is the call back number left on the voicemail stating they were the Indiana State Department of Health conducting a study on the health of Indiana residents. However, I could not find this number anywhere on their website, so did not call it back.
