833-691-4014 / 8336914014

I found out from my phone provider that ALL NUMBERS STARTING 833-691-XXXX ARE INVALID/SPOOFED. There is apparently no such combination of 833 (toll-free area code) and prefix 691. These are SCAMMERS who've bought stolen insurance info from multiple insurance companies and third-parties of insurance companies. They likely will ask you MANY questions to steal your info....DO NOT GIVE THEM ANY INFO. They pretend to be Anthem, Aetna, Inovalon (another super-annoying scammish, invasion of privacy company that "reviews records" for all private insurers), Carelon/Elevance (part of Anthem), and United Health....and likely more. I've personally been getting calls from 833-691-4013, 4014, and 4015. Just block them if your phone allows you to (mine wouldn't because they are "invalid numbers" ). I created names for them though, and avoid those ac/prefix combos like the PLAGUE that they are.
